Intermittent fasting: Benefits, how it works, and is it right for you?

Intermittent fasting is likely something you’ve tried or heard of a friend trying. This weight-loss trend continues to gain a lot of attention for people looking to slim down. UC Davis Health Registered Dietitian Melinda Gong explains intermittent fasting and some of the pros and cons of the eating plan.

Where did fasting originate?

To clarify some definitions, fasting is purposely not eating food. In contrast, starvation is not being able to eat due to factors out of your control, such as food insecurity.

The origins of the practice of fasting date back to ancient times where it was a common practice to fast to help heal the body. It also has roots in religion where it’s said to help deepen your connection to your faith. One example is during the holy month of Ramadan where fasting is encouraged from dawn to dusk.

How does intermittent fasting work?

The idea behind intermittent fasting is that by restricting food, our bodies will more quickly and efficiently tap our fat stores for energy. While glucose from carbohydrates is our most direct fuel source, we burn fat for energy when glucose isn’t available. This happens even more during times of food deprivation. Our ability to store fat is unlimited.

Intermittent fasting can be done in a variety of ways. There’s no “perfect” fasting plan, and it really should be based on what works best in your life.

One method is fasting daily for a set amount of time, usually 12 hours or more. The average person sleeps about 7 hours a night, which counts towards that fasting time. If you don’t eat after dinner, then you could easily achieve a daily fast to help your body burn fat more efficiently. This type of fasting could work well for someone who tends to be a nighttime snacker.

Another form of intermittent fasting is alternate day fasting. This is where you eat whatever you want 5 to 6 days of the week and choose 1 or 2 days a week to fast. During those fasting days, water and broth is encouraged so you don’t become dehydrated. This type of fasting could be beneficial for someone with a work schedule that may be really busy some days and cannot eat on a regular basis.

Is intermittent fasting right for you?

That question is difficult to answer. Overall, it’s important to determine why you want to try intermittent fasting, and what you hope to achieve. Is this something you plan to do forever?

Many people try fasting to lose weight, but if this isn’t something you can do forever – and if the plan isn’t sustainable – then you may regain that weight. Intermittent fasting can help with weight loss because it may help you eat less overall. This is especially true if you eat sensible portions and choose balanced meals during non-fasting times.

Take a moment to think about your eating habits. If you notice that you tend to snack a lot at night, intermittent fasting could give you a cut-off time and help you eat less. If you notice your last meal of the day is late in the evening, maybe you can eat earlier to allow your body to start fasting at that time.

What are other health benefits for intermittent fasting?

There is still not enough research on whether intermittent fasting can help manage chronic conditions. If you’re thinking of intermittent fasting, check with your health care team first. Any type of fasting could be dangerous if you’re on certain medicines or have certain conditions. A health care provider can review your health history and provide you with guidance.

It’s important to try methods that make you happy and don’t cause your mind and body harm. If following a set of rules of when you eat seems stressful, then intermittent fasting may not be right for you.

Að fasta er ein­fald­lega að sleppa því að borða. Oft­ast er mælt með að taka föstu þar sem þú drekk­ur bara vatn og ekk­ert annað í til­tek­inn tíma.

Kaffi og jurta­te hafa yf­ir­leitt ekki mik­il áhrif á ávinn­ing föst­unn­ar en aðrir drykk­ir sem inni­halda sætu- og bragðefni draga úr ávinn­ingi föstu, sömu­leiðis tyggjó vegna sætu­efna. Þurr fasta er bara nauðsyn­leg fyr­ir aðgerðir og slíkt en er mun erfiðari og óþörf fyr­ir flesta og ekki rædd hér.

Hversu oft er fastað og hve lengi í einu hef­ur mikið að segja um ávinn­ing föst­unn­ar og hvernig kerfi lík­am­ans bregðast við henni.

Af hverju að fasta?

Manns­lík­am­inn er hannaður til að þríf­ast við mis­jafn­ar aðstæður. Við vorum upp­haf­lega frum­byggj­ar sem þurftu að veiða sér til mat­ar og lifa eft­ir aðstæðum um­hverf­is­ins. Genin okk­ar hafa ekk­ert breyst þrátt fyr­ir land­búnaðar- og iðnaðarvæðingu sem hef­ur gjör­breytt neyslu­mynstri og lífs­stíl okk­ar.

Orku­kerfi lík­am­ans eru þannig upp byggð að við geym­um orku fyr­ir harðindi og skort og það er bein­lín­is óhollt fyr­ir okk­ur að neyta mat­ar á 2-3 tíma fresti all­an okk­ar vöku­tíma. Melt­ing­ar­kerfið þarf stund­um hlé svo lík­am­inn geti losað sig við óæski­leg efni, losað bólg­ur og end­urstillt sig. 

Hér skulu tald­ir upp 10 kosti þess að inn­leiða föstu í lífs­stíl­inn:

1. Styrk­ir hug­ann og vilj­ann

Efldu vilja­styrk­inn með föstu, finndu að þú get­ur lifað án mat­ar í smá tíma og liðið vel. Ögraðu þæg­ind­aramm­an­um með því að fasta, það eyk­ur sjálfs­traustið og auðveld­ar þér að ögra sjálf­um þér á fleiri sviðum. Í föstu eiga marg­ir einnig auðveld­ara með að hug­leiða eða finna and­leg­ar teng­ing­ar sem er ástæðan fyr­ir að í mörg­um trú­ar­brögðum er fast­an hluti af heil­ögu ferli. 2.  Breyttu lík­am­an­um í fitu­brennsluVÉL

Það eru mjög marg­ir sem bæta föstu í lífs­stíl­inn hjá sér til að brenna meiri fitu. Það er margt í föst­unni sem ýtir und­ir brennslu og þá sér­stak­lega þegar glúkósa­birgðirn­ar þverra og lík­am­inn skipt­ir um orku­gjafa og fer að nota fitu sem orku.

3. Meiri orka og vellíðan

Með því að gefa melt­ing­unni smá frí hef­urðu mun meiri orku til reiðu fyr­ir heil­ann og aðra starf­semi. Flest­ir sem hætta að borða eft­ir kvöld­mat og fram að há­degi dag­inn eft­ir upp­lifa mik­inn skýr­leika í hugs­un og finna fyr­ir auk­inni orku á meðan fastað er.

4. Minnk­ar bólg­ur

Bólg­ur (e. in­flamm­ati­on) eru eðli­leg­ar í lík­am­an­um til dæm­is í meiðslum. Þá eyk­ur lík­am­inn vökv­asöfn­un á því svæði til að verja skaddaðan vef, losa niður­brots­efni og flytja nauðsyn­leg efni á svæðið til að flýta end­ur­bata. Stöðugar og end­ur­tekn­ar bólg­ur í liðum, melt­ingu, kring­um taug­ar og í líf­fær­um er hins­veg­ar ekki eðli­legt og æski­legt ástand.

Allt sem þú get­ur gert til að draga úr bólg­um í lík­am­an­um hjálp­ar þér að auka vellíðan og draga úr veik­inda- og sjúk­dóms­hættu. Þar með talið eru leiðir eins og að velja hreint og nátt­úru­legt mataræði, fara í köld böð og stunda hreyf­ingu. Þegar þú fast­ar dreg­ur lík­am­inn úr los­un frumna sem kall­ast ein­kjörn­ung­ar (e. monocyte) í blóðið sem stuðla að bólg­um. Þess­ar frum­ur fara í hálf­gerðan dvala meðan fastað er, hvort sem um lotu-föstu er að ræða eða lengri föst­ur með reglu­legu milli­bili.

5. Betri efna­skipti

Efna­skipti (e. meta­bol­ism) eiga sér alltaf stað í lík­am­an­um og þau hægj­ast með aldr­in­um. Hraði efna­skipta í lík­am­an­um stjórn­ar því meðal ann­ars hvernig lík­am­inn vinn­ur úr ork­unni í lík­am­an­um. Í föstu hæg­ir lík­am­inn á efna­skipt­um en á sama tíma örv­ar það afl­gjafa frumna, hvat­ber­ana (e. mitochondria), til að fram­leiða meiri orku. Föst­ur virðast helst hafa þau áhrif að betra jafn­vægi kemst á efna­skipti lík­am­ans.

Ef þú borðar færri hita­ein­ing­ar en þú brenn­ir þá hæg­ir lík­am­inn á efna­skipt­um en á sama tíma verða þau skil­virk­ari og þá nýt­ist meira af þeirri nær­ingu sem þú inn­byrðir og sóar minna af ork­unni sem þú neyt­ir og geym­ir minna af henni sem vara­forða. Með því að fasta ertu að auka sveigj­an­leika efna­skipt­anna (e. Meta­bolic flex­i­bility) sem hef­ur marga kosti.

6. Frumu­til­tekt – út með ruslið

Sjálfs­át frumna (e. autoph­agy) er nátt­úru­legt ferli frumna sem felst í því að frum­ur losa sig við óþarfa og óvirka hluta með hjálp svo­kallaðra leysiagna (e. lyosome) sem nota ensím til þess­ara verka. Með aldr­in­um stífl­ast þess­ar leysiagn­ir upp svo mikið að ensím­in ná ekki að brjóta niður þessi úr­gangs­efni og þá helst á svæðum eins og í hjarta, augn­botn­um, tauga­frum­um og hjá öðrum frum­um sem skipta sér lítið eða ekk­ert. Með tím­an­um æxl­ast þetta svo í ýmsa öldrun­ar­sjúk­dóma. Með því að fasta örv­ast þessi starf­semi og fer á fullt eft­ir 24 tíma föstu og er orðin um 300% meiri eft­ir 36 tíma föstu og nær há­marki eft­ir 72 tíma.

7. Ketó-kraft­ur!

Skil­virk­asti og aðgengi­leg­asti orku­gjaf­inn sem lík­am­inn not­ar er blóðsyk­ur/​glúkósi (e. glucose). Þegar þú borðar kol­vetni los­ar lík­am­inn insúlín sem bind­ur glúkósa og geym­ir hann í lifr­inni og í vöðvun­um sem glýkógen. Heil­brigður lík­ami reyn­ir að halda blóðsykr­in­um í kring­um 4-5 mmol/​l og not­ar þetta kerfi til að viðhalda hon­um þannig. Þegar þú fast­ar byrj­ar lík­am­inn að leysa út þess­ar glýkógenbirgðir í blóðið aft­ur til að stjórna blóðsykr­in­um og reyna að halda hon­um í jafn­vægi. Þess­ar birgðir duga yf­ir­leitt í kring­um sól­ar­hring.

Það sem ger­ist þegar glýkógen birgðirn­ar klár­ast og blóðsyk­ur­inn dett­ur niður fyr­ir 4 mmol/​l skipt­ir lík­am­inn um orku­gjafa. Þá brýt­ur lík­am­inn niður fitu og los­ar ketóna sem orku­gjafa sem er ein­stak­lega skil­virk­ur orku­gjafi fyr­ir heil­a­starf­semi til dæm­is og þess vegna upp­lifa marg­ir auk­inn skýr­leika og meiri orku.

Það má líkja sykri sem orku­gjafa við að brenna þurrt hey meðan fita er eins og að brenna kol­um. Syk­ur og ein­föld kol­vetni gefa mikla orku hratt en brenna fljótt upp og kalla á stöðugt meira meðan fita er leng­ur að byrja að mynda orku en er jafn­ari og end­ist mun leng­ur. Þess vegna finn­ur þú fyr­ir minni svengd þegar fita er þitt eldsneyti og sveifl­urn­ar í blóðsykri verða mun minni.

8. Góð áhrif á horm­óna­kerfið

Syk­ur­sýki 2 (insúlí­nó­háð syk­ur­sýki) er áunn­inn sjúk­dóm­ur sem verður oft­ast til þegar lík­am­inn miss­ir stjórn á insúlín los­un og nær ekki leng­ur að binda glúkósa. Við það hækk­ar blóðsyk­ur­inn og viðkom­andi get­ur hækkað mjög í blóðsykri sem er mjög skemm­andi ástand og get­ur verið lífs­hættu­legt. Syk­ur­sýki 1 (insúlí­n­háð syk­ur­sýki) er oft­ast meðfædd og veld­ur því að lík­am­inn fram­leiðir ekki nóg af insúlíni með svipuðum af­leiðing­um.

Föst­ur eru senni­lega besta leiðin til að lækka fram­leiðsluna á insúlín horm­ón­inu og reglu­leg­ar föst­ur eru besta leiðin til að auka insúlín næmi lík­am­ans.

Vaxt­ar­horm­ón (e. Growth horm­o­ne) fram­leiðsla örv­ast við lengri föst­ur. Með aldr­in­um minnk­ar los­un vaxt­ar­horm­óns, en það gegn­ir mik­il­vægu hlut­verki í úr­vinnslu á fitu og orku. Það hjálp­ar einnig við að viðhalda vöðvamassa og beinþéttni þar sem það er mik­il­vægt í end­ur­nýj­un frumna.

Noradrenalín (e. noradrena­line/​nor­ep­inephrine) er streitu­horm­ón sem eyk­ur ár­vekni og at­hygli. Þetta horm­ón hef­ur mörg önn­ur áhrif á lík­amann, þar á meðal að segja fitu­frum­um lík­am­ans að losa fitu­sýr­ur sem orku­gjafa. Lang­ar föst­ur leiða til hærra magns noradrenalíns í blóðinu og þar kem­ur veiðimann­seðlið okk­ar inn, svo­kallað „fig­ht or flig­ht“ viðbragð örv­ast.

10. For­vörn gegn sjúk­dóm­um og lengra líf!

Þegar við verðum veik þá minnk­ar mat­ar­lyst­in okk­ar og lík­ams­hit­inn hækk­ar. Það er ástæða fyr­ir því, lík­am­inn get­ur varið ork­unni í að berj­ast við sýkl­ana í stað þess að melta mat og hit­inn ger­ir um­hverfið ólíf­vænna fyr­ir bakt­erí­ur. Föst­ur hafa mikið for­varn­ar­gildi gegn veik­ind­um og sjúk­dóm­um af áður upp­töld­um ástæðum. Þegar þú fast­ar þá minnk­ar magn hvítra blóðkorna (e. lymp­hocytes) sam­hliða ann­arri til­tekt í frumun­um og stofn­frum­urn­ar fara þá á fullt í að end­ur­nýja hvít­korna­birgðirn­ar og end­ur­nýja ónæmis­kerfið. 

Bara það eitt að tæma ristil­inn af og til get­ur komið í veg fyr­ir sjúk­dóma vegna út­hreins­un­ar­inn­ar. Þarma­flór­an nær betra jafn­vægi þegar þess­ari til­tekt er lokið.

Auk­inn efna­skipta­leg­ur sveigj­an­leiki, minni bólg­ur, betra insúlín-næmi, til­tekt í frum­um, betri þarma­flóra, sterk­ara ónæmis­kerfi og allt það sem upp er talið stuðlar að betri heilsu í lengri tíma. Heil­brigður lífs­stíll með reglu­leg­um föst­um ætti að auka lík­ur okk­ar á að lifa við betri heilsu í lengri tíma.

Að brjóta föstu

Þegar þú fast­ar breyt­ist lík­ams­starf­sem­in aðeins. Þú verður viðkvæm­ari fyr­ir kol­vetn­um strax eft­ir föstu þar sem þau hækka insúlínið hratt. Hóf­leg­ur skammt­ur af pró­tíni eins og úr laxi eða kjúk­lingi skaff­ar gott magn af nauðsyn­leg­um fitu­sýr­um. Marg­ir mæla með kjöt- eða beina­soði til að brjóta föstu vegna nær­ing­ar­gild­is­ins.

Borðaðu smáa skammta og passaðu að tyggja mat­inn mjög, mjög vel. Lág­kol­vetna mataræði hent­ar vel og viðheld­ur mörg­um kost­um föst­unn­ar áfram.

Al­geng­ar hindr­an­ir

Erfiðasti tím­inn er helst á meðan mag­inn er að minnka og kerfið er að tæma sig og þegar blóðsyk­ur­inn lækk­ar og lík­am­inn skipt­ir um orku­gjafa. Að hugsa stöðugt um mat er vanda­mál fyr­ir marga og því betra að hafa eitt­hvað fyr­ir stafni á föstu. Svo get­ur kaffi eða te mildað um­skipt­in ef þú ákveður að leyfa þér það.

Orku­skipt­in frá glúkósa yfir í fitu get­ur kallað fram flensu-lík ein­kenni (e. carb-flu/​keto-flu) eins og höfuðverk, þreytu, sljó­leika, vöðvakrampa og bein­verki. Hægt er að minnka þessi ein­kenni með því að drekka nóg af vatni og bæta við ögn af salti í vatnið af og til. Þessi um­skipti eru erfiðari ef mataræðið er venju­lega mjög kol­vetnaríkt og fitusnautt og blóðsyk­ur­inn er í hærri kant­in­um. Með því að trappa upp hversu lengi þú fast­ar eru áhrif­in einnig væg­ari, til dæm­is ef þú hef­ur fastað 16:8 í ein­hvern tíma og tek­ur svo lengri föstu.

Pæl­ing­ar

Mikl­ar rann­sókn­ir hafa verið gerðar um föstu síðastliðin ár og stund­um stang­ast niður­stöður á. Einnig eru marg­ar kenn­ing­ar í gangi sem eru ekki endi­lega byggðar á rann­sókn­um og úr­tök­in eru vissu­lega mis­stór, allt frá ein­stak­ling­um sem eru í til­rauna­starf­semi á sjálf­um sér yfir í stóra hópa yfir lang­an tíma. Hvernig þess­ar rann­sókn­ir eru fram­kvæmd­ar er einnig mjög breyti­legt, sumt á til dæm­is við um lotu­föst­ur og annað á við um lengri föst­ur. Við þessi greina­skrif var farið í marg­ar djúp­ar kan­ínu­hol­ur og reynt að draga lest­ur­inn sam­an í stutt­ar, ein­fald­ar máls­grein­ar.
